What exactly is skin botox?
Skin botox (microbotox, mesobotox, 스킨보톡스, スキンボトックス, 水光肉毒, 微滴肉毒) is the injection of diluted botulinum toxin type A into the dermal layer of the facial skin, delivered as very small droplets across many points. Unlike conventional botox, which goes deep into the muscles that create wrinkles, skin botox stays shallow, acting only on sebaceous glands, the tissue around pores, and the uppermost fibres of the fine expression muscles.
That difference shows in the result. If conventional botox is about "wrinkles smoothing out", skin botox is closer to "skin looking refined and less shiny". It is often chosen by people who have hesitated over botox because they worried about looking unnatural.

How it works, and what the research shows

The same drug acts on entirely different structures depending on injection depth.
Botulinum toxin blocks the release of acetylcholine, the signalling molecule nerves use to instruct tissue, at the nerve ending. Injected into muscle, the muscle relaxes. Injected shallowly into the dermis, other structures within the skin are affected instead.
First, the sebaceous glands. These are under cholinergic nerve control, so blocking that transmission reduces oil production. A 2021 review in Toxins set out this mechanism and examined the evidence for use in seborrhoea and acne, and a 2022 cell study reported that picomolar concentrations of the toxin directly modulate lipid production in sebocytes themselves. In other words, a direct effect on the oil-producing cells may contribute alongside the nerve pathway.
Second, the arrector pili muscles, the small muscles attached to follicles that raise goose bumps. When these relax, the pull on the pore opening decreases and pores appear less open.
Third, the superficial fine expression muscles. Only the shallow fibres just beneath the skin are partially softened, so fine lines soften while larger expressions are preserved.

One drug acting on three axes at once: oil, pores and fine lines.
The most frequently cited clinical evidence is Shah (2008, Journal of Drugs in Dermatology). Of 20 patients given intradermal toxin, 17 reported reduced sebum production at one month, and 17 reported reduced pore size. In 2023 the Aesthetic Surgery Journal published an expert roundtable on intradermal microdosing for pores, skin laxity, oil control and scarring, reflecting how clinical protocols are becoming standardised.

The session, step by step
After cleansing, numbing cream is applied for 20-30 minutes, then a very fine needle is used to inject small amounts across the whole face or over chosen zones such as the T-zone and cheeks. The injections take 15-20 minutes. Fine red dots are visible immediately afterwards but usually settle within a few hours. You can cleanse as usual that evening and wear makeup the next day.
Aftercare
- Do not lie down for four hours, and do not rub or massage the treated area, to keep the drug from spreading into unintended layers.
- Avoid saunas, steam rooms, vigorous exercise and alcohol for 24 hours.
- Effects emerge gradually from 3-7 days and peak around two weeks, so it is best not to conclude it has not worked before then.
- When the effect fades after 3-4 months, repeat treatment maintains it.
