What exactly is LDM?
LDM (Local Dynamic Micromassage, water-drop lifting, 물방울 리프팅, 水滴提升, 水玉リフティング, dual/triple-frequency ultrasound) is a high-frequency ultrasound device developed by Wellcomet in Germany. Unlike HIFU, which coagulates tissue with heat, LDM does not burn the skin; it adjusts the cellular environment. During treatment you only feel a handpiece gliding over warm gel, which is why it is often called a treatment you can rest through.
Ultherapy and Shrink are also ultrasound, but they create thermal coagulation points deep in the skin, a strong stimulus. LDM is closer to conditioning: it raises the baseline state of the skin without irritation. That is why it can be used on sensitive skin, freshly treated skin and breakout-prone skin.

Mechanism and clinical evidence
The higher the ultrasound frequency, the shallower it is absorbed. Energy at 1 MHz reaches the subcutaneous fat, 3 MHz reaches the deep dermis, and 10 MHz concentrates in the epidermis and upper dermis. LDM switches between these frequencies hundreds to thousands of times per second, creating tiny pressure changes in the tissue. This is, quite literally, a local dynamic micromassage.
The changes this produces in the skin are described in three parts.
- Regulating MMPs (matrix metalloproteinases, the enzymes that break down collagen): lowers MMP activity that is excessive in inflamed and aging skin, reducing collagen breakdown and inflammation.
- Raising HSPs (heat shock proteins, protective proteins that repair damaged proteins): helps new collagen form properly and supports tissue recovery.
- Improving the GAG environment (glycosaminoglycans, water-binding molecules such as hyaluronic acid): relates to the ability of the dermis to hold water.
There is clinical evidence as well. In a multicentre study by Meyer-Rogge et al. (2012, Journal of Cosmetics, Dermatological Sciences and Applications), 39 people aged 30–63 received 3/10 MHz dual-frequency ultrasound twice a week for 8–12 sessions. Coarse wrinkles improved by an average of 0.63 grades, fine wrinkles by 0.73 grades and pores by 0.65 grades (all p < 0.05). Patient satisfaction was 6.87 out of 10.
Four benefits: calming, repair, acne, lifting
1. Calming
Redness and a burning feeling mean inflammatory signals are switched on in the skin. Because LDM regulates MMPs without thermal damage, it is one of the few devices that can be used when the skin should not be stimulated further, such as right after a laser or peel, or on seasonally sensitive skin.
2. Repair
Higher HSP levels and better microcirculation are involved in how quickly a damaged skin barrier recovers. This is why LDM is added after other procedures, to shorten the period when the skin is reactive.
3. Acne
Chervinskaya et al. (2025, Journal of Cosmetic Dermatology) treated 24 patients with mild-to-severe acne using triple-frequency LDM alone, without medication, for up to 12 sessions over 6 weeks. The acne severity score (mGEA) fell by 73.7% at the end of treatment and by 90.1% at follow-up, and side effects were limited to temporary redness that faded within an hour. It is a small retrospective study, but it supports considering LDM as an adjunct for acne patients who cannot take medication or whose skin is reactive.
4. Lifting (firmness)
Despite the name, LDM does not pull up sagging tissue. It reduces collagen breakdown and replenishes water in the dermis so the skin looks plumper, which means better firmness and fewer fine lines. If your main concern is a sagging jawline or cheeks, HIFU or radiofrequency lifting comes first, and LDM supports that result.
Why it is especially good for hydration
Skin that is shiny on the surface yet tight underneath, often called inner dryness, rarely improves by layering more skincare. The water is held not in the outer layer but by GAGs in the dermis.
LDM suits hydration care for three reasons. First, 10 MHz ultrasound concentrates in the epidermis and upper dermis, acting directly on the water-holding environment. Second, it creates no heat or wounds, so it does not burden dry skin with a weakened barrier. Third, moisture and glow can be felt right after treatment, making it a good choice before an important event.

Frequently asked questions
Q1. How many LDM sessions do I need to see results?
Hydration and calming are often felt right after the first session. The clinical study that confirmed improvement in wrinkles and pores, however, used a protocol of twice a week for 8–12 sessions. If firmness and texture are your goal, a series is better.
Q2. LDM and Ultherapy are both ultrasound. What is the difference?
Ultherapy and Shrink are HIFU: they focus ultrasound on one point to create thermal coagulation above 60°C. LDM alternates frequencies to regulate the cellular environment without thermal damage. The former targets sagging; the latter targets calming, hydration and texture.
Q3. Can I have LDM while I have active acne?
Yes. A reduction in severity has been reported in inflammatory acne (Chervinskaya et al., 2025), and the low level of irritation makes it gentle on breakout-prone skin. Severe pustular acne may still need medication, extraction or other treatment alongside.
Q4. Can I wear makeup or exercise on the day of LDM?
Yes. It does not wound the skin, so there are no restrictions on washing, makeup or daily activity afterward. If you had another procedure such as a laser on the same day, follow the aftercare for that procedure.
Q5. Is LDM possible with sensitive skin or facial redness?
Yes, and it is often a first choice for skin that needs to avoid irritation. Temporary redness can occur but usually settles within an hour.
