What are the three treatments?
- Re2O (Elravie Re2O, 리투오, hADM skin booster, phADM, ECM skin booster) is an injection of micronized extracellular matrix (ECM, the collagen and elastin scaffold inside the skin) prepared from decellularized human dermis and placed into the dermis. It is about 80% collagen and 3% elastin. It is not a medical-device filler; it is a human tissue product regulated by Korea's Ministry of Food and Drug Safety.
- Filler (HA filler, dermal filler, 필러, ヒアルロン酸フィラー, 玻尿酸填充) is a cross-linked hyaluronic acid gel placed in the deeper layers under the skin to fill a hollow space directly.
- Sculptra (스컬트라, スカルプトラ, 塑妍萃, 舒顏萃, PLLA collagen biostimulator) is an injection of PLLA (poly-L-lactic acid, a polymer the body slowly breaks down) particles that prompt your own collagen to form around them.

Where does Re2O's volume come from?
The volume Re2O creates is the skin itself becoming plumper. Think of a sagging mattress: filler and Sculptra refill the inside, while Re2O thickens the worn outer fabric.
The injected ECM particles act as a framework that cells move into. In an animal experiment, fibroblasts (the cells that make collagen) had entered the particles one week after injection, and the most new collagen formed at the boundary where the particles meet the host tissue. Because the volume comes from dermal tissue filling in rather than from a gel occupying space, the change is gradual and natural. For the same reason, Re2O is weak at building shape, such as raising the nose bridge or projecting the chin.
What does the evidence say?
Re2O. A randomized, double-blinded trial by Lee et al. (2026, International Journal of Molecular Sciences) injected Re2O with hyaluronic acid into one side of the face of 20 adults (mean age 54.7) and a hyaluronic acid skin booster alone into the other side, then followed them for 20 weeks. Each treated side received about 56 mg of Re2O. Skin volume measured by 3D imaging increased more on the Re2O side at every time point, and skin density measured by ultrasound also improved more at week 20. There were no serious adverse events. Three cautions apply when reading this result. The control was a non-cross-linked hyaluronic acid skin booster, not a volumizing filler. Follow-up ended at 20 weeks, so durability beyond that is unknown. One of the authors served as an advisor on the product's development. This study cannot be used as evidence that Re2O builds as much volume as filler.
Acellular dermal particles used as a filler. Zhang et al. (2026, Aesthetic Plastic Surgery) compared a porcine dermis-derived particle filler with a collagen filler. Among 202 patients with moderate to severe nasolabial folds, the share with improved wrinkle grades was similar: 88.4% versus 85.4% at 3 months and 70.9% versus 69.7% at 6 months. However, 69.8% received a supplemental injection at 6 weeks, and pain (40.7%) and swelling (45.4%) were more common than with the control. That product differs from Re2O in source material, particles and injection layer, so its results cannot be read across to Re2O.
Sculptra. A randomized trial by Narins et al. (2010, Journal of the American Academy of Dermatology) treated 233 people with up to 4 sessions at 3-week intervals. Correction of nasolabial folds was maintained up to 25 months after the last session, and it took about 6.5 months on average to reach peak correction.
As of October 2026, no clinical trial directly comparing Re2O with filler or Sculptra has been published.
When Re2O can stand in, and when it cannot
The answer depends on whether the hollow look comes from the thickness of the skin or from the fat and bone beneath it.
When Re2O is an alternative
- Eye area and cheeks that look sunken along with fine lines because the skin has thinned
- Shallow nasolabial folds and lines around the mouth
- Wanting a gradual change because of concern about a foreign-body feel or overcorrection with filler
- When skin density, elasticity and pores matter as much as volume
When filler or Sculptra fits
- Structural hollowing from fat and bone loss, such as the front cheeks, temples and chin
- Areas that need shaping, such as the nose, jawline and lips
- When a visible change is needed the same day (filler)
- When the whole face has hollowed broadly and needs soft, even filling (Sculptra)
Will Re2O V replace filler?
On October 7, 2026, Re2O V, a new addition to the Re2O line, was unveiled at an academic event in Seoul. According to the manufacturer, its particles are more than twice the size of the original Re2O and the dose is raised to 300 mg, aiming at larger defects and deeper areas that need structural support. If Re2O and Re2O Fine handled concerns close to the skin surface, Re2O V takes a step toward the territory that filler and Sculptra have covered.
What has been shared so far is the product design and physicians' early experience. There are no clinical data comparing it with filler or Sculptra yet, so whether it can replace them has to be judged from research still to come.
How does this fit a trip to Korea?
- Filler lets you see the result right after treatment and fly home.
- Re2O is planned as a single session when you are visiting once. In our head doctor's clinical experience, many patients see a change after one session, though results vary.
- Sculptra is normally given in several sessions weeks apart and shows its result months later, so it suits those who plan to return.
- Having treatment early in your trip lets swelling settle during your stay and allows a check before departure. Avoid saunas, strenuous exercise and alcohol for 2–3 days afterward.
At DIORE
At DIORE, Re2O is often combined on the same day with device treatments such as Ultherapy, Thermage and Potenza. They act on different layers, so the skin is not overloaded, and the approach suits international patients who need results from a single visit. On a combined day, the device treatments come first and Re2O last.
Frequently asked questions
Q1. Can Re2O alone fill hollow cheeks?
If the cheeks look hollow because the skin has thinned, you can expect them to look plumper. Deep hollowing from fat and bone loss is not filled by Re2O alone. Which cause applies is determined at consultation.
Q2. Can I get Re2O in an area that already has filler?
Filler sits in the deeper layers under the skin and Re2O in the dermis, so the injection layers differ. Their roles differ as well, so they can be planned together. Whether to combine them on the same day, and the interval, is decided at consultation depending on the area and the type of filler.
Q3. Which lasts longer, Sculptra or Re2O?
The current evidence does not allow a direct comparison. Sculptra has a trial with 25 months of follow-up, while Re2O has only a trial confirmed up to 20 weeks. That does not mean Re2O is short-lived; it means long-term data do not exist yet.
Q4. Re2O is human tissue. Is it safe?
Re2O goes through donor screening and processing under American Association of Tissue Banks (AATB) guidelines, and cellular components are removed so that residual DNA is below 50 ng per mg. In the 2026 trial there were no serious adverse events, and transient redness and slight swelling resolved on their own. With only 20 participants, the study could not rule out rare adverse events.
Q5. I am in Korea for only a few days. Which volume treatment should I get?
If you need a change in shape the same day, filler fits. If you also want to improve skin thickness and texture, Re2O fits. If you have both concerns, they are planned separately. Sculptra is worth considering when your schedule allows a return visit.
Summary
Re2O does not take the place of filler and Sculptra; it fills the thickness of the skin itself, which those two could not. If thinned skin is the issue, Re2O may be a better answer than filler, and if the structure has hollowed, filler or Sculptra is still needed. With larger-particle products such as Re2O V the boundary is moving, but on current evidence this is a division of roles, not a replacement.
